VISTA Expression May Help Predict Survival Outcomes in Mesothelioma Patients Treated with Nivolumab

A new study from Yokouchi and colleagues is shedding light on why some patients with pleural mesothelioma respond better to immunotherapy than others. Researchers from Japan have identified a potential biomarker, known as VISTA (V-domain immunoglobulin suppressor of T-cell activation), that may help predict survival outcomes in patients treated with nivolumab.

Their findings suggest that tumor-cell VISTA expression could play an important role in guiding future treatment decisions for this difficult-to-treat cancer.

Researchers Search for Better Mesothelioma Biomarkers

Pleural mesothelioma remains an aggressive cancer with limited treatment options, particularly after first-line therapy fails. While immune checkpoint inhibitors such as nivolumab have shown promise in extending survival for some patients, responses are often unpredictable.

Clinicians have long sought reliable biomarkers that can identify which patients are most likely to benefit from immunotherapy. This study represents an important step toward that goal.

The researchers conducted a retrospective, multi-institutional analysis involving 55 patients with pleural mesothelioma who received nivolumab as a second-line or later treatment at 18 hospitals across Japan. Tumor samples collected before first-line treatment were analyzed using immunohistochemistry to evaluate protein expression, while genomic testing was performed on a subset of patients using next-generation sequencing.

The investigators then examined whether specific biomarkers were associated with progression-free survival and overall survival after nivolumab treatment.

High VISTA Expression Linked to Improved Survival

Their most notable finding involved VISTA expression on tumor cells. VISTA is an immune-regulatory protein that has drawn increasing attention in cancer research because of its role in controlling immune responses within the tumor microenvironment.

In this study, the researchers classified tumors as having either high or low VISTA expression using a cutoff of 10% VISTA-positive tumor cells. Among the 42 patients with immunohistochemistry results:

• 35 patients had high VISTA expression

• 7 patients had low VISTA expression

The differences in outcomes between these groups were significant.

Patients whose tumors demonstrated high VISTA expression experienced a median progression-free survival of 5.1 months, compared with 2.4 months for patients with low VISTA expression.

High VISTA expression was also associated with improved overall survival:

• 12.8 months median overall survival for patients with high VISTA expression

• 4.3 months median overall survival for patients with low VISTA expression

These findings suggest that VISTA may serve as a meaningful prognostic marker for patients receiving nivolumab therapy.

VISTA Remained Significant After Adjusting for Other Factors

Importantly, the association remained significant even after researchers adjusted for other clinical variables.

Multivariate analysis identified high tumor-cell VISTA expression as an independent predictor of both longer progression-free survival and improved overall survival. This means the survival benefit could not simply be explained by differences in patient demographics, disease severity, or other clinical characteristics.

Instead, VISTA itself appeared to have independent prognostic value.

What Makes VISTA Different?

The findings raise important questions about the biology of mesothelioma and the complex relationship between tumors and the immune system.

In many cancers, immune checkpoint molecules are associated with immune suppression and poorer outcomes. However, mesothelioma may behave differently. Previous research has suggested that VISTA is highly expressed in mesothelioma compared with many other malignancies.

The current study indicates that higher VISTA expression may identify a subgroup of patients who are more likely to benefit from nivolumab treatment, making it a potentially valuable biomarker for future treatment planning.

More Research Is Needed

While the results are encouraging, the authors caution that further research is necessary before VISTA testing becomes part of routine clinical practice.

Several limitations should be considered:

• The study was retrospective.

• The patient population was relatively small.

• All participants received nivolumab, making it difficult to determine whether VISTA specifically predicts response to nivolumab or simply reflects a less aggressive disease biology.

Prospective studies with comparison groups will be needed to validate these findings and clarify the role of VISTA as both a prognostic and predictive biomarker.

Precision Medicine and the Future of Mesothelioma Treatment

The study highlights the growing movement toward precision medicine in mesothelioma care.

Rather than relying solely on clinical characteristics, researchers are increasingly investigating molecular and immune-based biomarkers that can help personalize treatment decisions. If future studies confirm these findings, VISTA expression could help physicians:

• Identify patients more likely to benefit from immunotherapy

• Guide treatment sequencing decisions

• Inform the use of nivolumab and other checkpoint inhibitors

• Support development of combination treatment strategies

• Improve individualized mesothelioma treatment planning

Although significant challenges remain, studies like this demonstrate continued progress in understanding the biological factors that influence treatment response.
